Preventing Hospitalizations - CHF Management

According to recent national studies, hospital readmission rates for Congestive Heart Failure are at 25% within 30 days of discharge.

Congestive Heart Failure is a serious chronic progressive condition that affects the pumping power of the heart muscles. While often referred to simply as “heart failure,” CHF specifically refers to the stage in which fluid builds up around the heart and causes it to pump inefficiently.

Pure Home Health Care works with the physician to reduce hospital readmission rates with our specialized CHF management program. The program allows our Nursing team to establish patient/caregiver specific education and interventions including Skilled Nursing phone follow up protocols that identify escalating CHF symptoms, and also provide coordination of care, to help improve overall patient outcomes and prevent rehospitalization.

Heart failure signs and symptoms may include:

  • Shortness of breath (dyspnea) when you exert yourself or when you lie down.

  • Fatigue and weakness.

  • Swelling (edema) in your legs, ankles, and feet.

  • Rapid or irregular heartbeat.

  • Reduced ability to exercise.

  • Persistent cough or wheezing with white or pink blood-tinged phlegm.

  • Increased need to urinate at night.

  • Swelling of your abdomen (ascites).

  • Very rapid weight gain from fluid retention.

  • Lack of appetite and nausea.

  • Difficulty concentrating or decreased alertness.

  • Sudden, severe shortness of breath and coughing up pink, foamy mucus.

  • Chest pain if your heart failure is caused by a heart attack.

Pure Home Health Care has been making strides in keeping our clients from readmission by using our CHF phone call protocol by:

  • Ensuring that our patients have working scales in their homes to monitor their weight every day.

  • Checking on our CHF patients by phone on the weekends to ensure they are not having any exacerbation of symptoms.

  • Establishing frequent Skilled Nursing visits and monitoring for any increased shortness of breath, weight gain, and proper medication administration.

  • Ensuring that patients are taking their medication/s correctly, understand the reasoning for this medication /s and if they are having difficulty obtaining their medication/s.

  • Educating patients to contact Pure Home Health Care with any health-related issues at any time and we will assess & contact the physician if needed.
